Keep in mind that teachers want you to succeed. They're not going to yell at you for having mental health problems (and the vagueness of the words "personal issues" could mean many things). They might tell your school's counselor, but it's easier to get help that way.
Anyways, here is how to complete the assignments themselves.
First, recognize that the assignments are usually not too hard. Realistically, if you can focus, most should only take like 20 minutes at most. If you have essays, you could probably write a sentence or two every minute, or a paragraph every 5.
Also, set realistic goals. If you haven't been focusing on things for a while, don't expect yourself to be able to concentrate for an entire morning. You've been practicing how to be distracted for so long, so you need to practice focus. Work for 5 minutes, and take a 1-minute break. Do this about 5 times. This should take 30 minutes, with 25 minutes of focus and 5 minutes of break. Then, work your way up. 10 minutes with 2 minutes of break. 15 minutes with 3 minutes of break. 20 with 4, and 25 with 5.

For breaks: DO NOT USE YOUR PHONE, PLAY VIDEO GAMES, ETC. It is too easy to pick it up, intending to take 1 minute, but then you spend an hour or two. Instead, walk around your house, drink some water, eat some almonds, or something. Don't cut into your work time by even 20 seconds to finish the last few seconds of a video.
If you have 5 classes of homework, you would have 30 assignments per class. And if each assignment takes about 20 minutes, then each class has 600 minutes or 10 hours of homework. You can probably manage to finish 30 assignments in a day or two.
Now, to prevent this from happening again/getting worse: PAY ATTENTION DURING CLASS. Do not watch videos, scroll through social media, or cook eggs. Assignments shouldn't be impossibly difficult if you take notes. Put your phone in another room- you don't need to talk to your friends or see your DMs while you're in math.
